Responsible for the case management of quality home care services rendered to patients. Current NYS RN license & graduate of accredited school of nursing required. BSN preferred. Min 1 year recent medical/surgical nursing exp required. Exp with MRDD patient and/or Public/Community Health preferred. Bilingual English/Spanish or Bilingual English/Chinese.
